The Rural Northwest Missouri Behavioral Health Consortium would like to gather information from the area communities. This survey takes a few minutes to complete and has 24 questions. The data collected will not be given to West Nodaway, but it will be used to determine necessary funding for behavioral health resources in the area. If you would like to participate on a voluntary basis, use the QR code to access the survey.
